Intermountain Health Remote Clinical Social Worker in Murray, Utah

Job Description:

The Connect Care- Behavioral Health Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, implement plans of care with the appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide individual treatment as indicated. This position works collaboratively with patients, their family and/or caregivers, and providers so they can self-manage their care, optimize their functional health status, have quality outcomes, and use the health system appropriately. Develop, document, and implement plans which will provide the appropriate resources to address social, physical, mental, emotional, spiritual, and supportive needs. Facilitate virtual group therapy.

  • FTE: Full time 40hrs.

  • Shift: Flexible shifts, one weekend day per month and one evening per week. required.

Position Details:

Clinic hours are 7a-7p, 7 days per week. Closed holidays. Flexible schedule, 40 hrs. per week.

This role will be a part of the Connect Care- Behavioral Health Team.

Looking for a candidate with experience in the treatment of adolescents and/or pain management.

Will accept LCSW or LMFT licensure.

Salary will be based on licensure and experience.

Minimum Qualifications

  • Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.

  • Current State Licensure as LCSW or LMFT.

  • Computer Skills, i.e., email, word processing.

  • Excellent written and verbal communication skills.

Preferred Qualifications:

  • Experience with patients, specifically adolescents and/or pain management, and treatment teams .

Physical Requirements:

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

  • May be expected to stand in a stationary position for an extended period of time.
