Intermountain Health Behavioral Health Therapist in Grand Junction, Colorado

Job Description:

This position utilizes substance use and mental health expertise to perform biopsychosocial assessments, treatment planning, education, crisis assessments to appropriately provide needed resources and or referrals for the patient, and to provide family, individual, and group treatment. The behavioral health provider works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

We offer outpatient integrated medical and behavioral health services for people and their loved ones whose lives have been interrupted by addiction and co-occurring mental health conditions. Your work would include behavioral health assessment and intervention, resource connection, addressing barriers to care and treatment, treatment adherence and monitoring among others in our Provider Addiction Clinic.

Qualifications:

  • Provide brief behavioral health support to persons living with addiction

  • Provide consistent therapy for patients and their families. This will include individual, couple, family, and group therapy.

  • Conduct comprehensive diagnostic behavioral health evaluations.

  • Monitor caseload for treatment progress or regression and update treatment plans as necessary in compliance with agency timelines.

  • Monitor adherence to medical and behavioral health treatment

  • Connect patients with community resources for biopsychosocial, financial, and other needs

  • Communicate and collaborate with medical, interdisciplinary team staff, patient / family, insurance companies, and social agencies.

  • Providetimely documentation of all services within the clinic’s electronic medical record

  • Provide discharge planning, aftercare recommendations, and provide aftercare referrals as appropriate.

  • Maintain compliance with agency policies/guidelines and BHAregulatory standards.

Required:

  • Masters degree in a related field

  • LMFT, LAC, LCSW and/or LPC

  • Three years of relevant work experience

Preferred:

  • EMDR trained

  • Five years of relevant work experience

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc). May be expected to stand in a stationary position for an extended period of time.
