Intermountain Health RN Care Manger Lead in Brighton, Colorado
Job Description:
This position provides clerical and administrative support to enhance integrated care management for targeted populations and individuals. Facilitates communication with payers and authorization of services in accordance with regulatory and contractual requirements, patient and family-centered interdisciplinary care planning, effective care coordination, collaboration and communication between healthcare settings, and effective transitions to the appropriate level of care.
As a Lead Case Manager, you need to know how to:
Lead the day to day work Implements directives related to adjusting staffing levels based on service needs and volumes.
Act as a resource.
Perform case management duties as required.
Participate and lead improvement initiatives.
Coordinate orientation and training of new associates.
Ensure staff competency through proper training techniques and conduct quality audits.
Participate in problem solving to address issues relating to patient safety, care and service.
Support and promote effective working relations and work effectively as part of a department/unit team and interdepartmentally to facilitate that departments ability to meet its goals and objective.
Collaborate with department leadership team to coordinate care with other departments and promote the highest level of patient care and satisfaction
May participate in human resource functions.
May participate in the interview process and provide feedback to leadership team on candidates.
Provide education/training and performance feedback for associates in the department.
Participate in policy and procedure development.
Present and document staff meetings as required.
Participate in leading quality and performance improvement initiatives.
Ensure compliance with applicable regulatory guidelines and established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
Coordinate the care and services for patients identified as needing assistance or meeting Care Management criteria.
Collect in-depth information about a patients medical, functional, and social condition to identify individual needs in order to develop a plan to meet those needs.
Work with patient and family/caregivers to determine specific goals and actions based on assessment.
Coordinate discharge planning.
Execute specific interventions to meet established goals.
Organize, integrate and coordinate the necessary resources to accomplish the goals and plan.
Assess the patients prior level of functioning, access to and/or use of community resources and available support systems.
Assist the care team in developing a plan of care which includes, but is not limited to: assuring appropriateness of services and care setting, assuring individualized support and education, determining the need for continued services, planning for discharge, and identifying and connecting patients/families with available community resources if needed.
Collaborate with Physicians and other members of the health care team on the patients behalf.
Identify appropriate admission and continued stay issues.
Enhance the quality of patient care through effective and efficient use of resources.
Help to identify strategies for reducing length of stay and appropriate utilization of services.
Utilize criteria including clinical pathway data and implement strategies to resolve controllable variances.
Attend, facilitate and participate in rounds and case conferences. Advocate for patient rights.
Monitor efficiency and availability of services and evaluate outcomes through variance tracking, data collection.
Identify needs, facilitates, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles.
Participate in multidisciplinary groups and development of guidelines.
Minimum Qualifications
Graduate of an accredited school of nursing (Bachelors Degree in Nursing (BSN) may be required for some care sites)
Current Colorado RN license or compact license with the ability to obtain Colorado RN license
Minimum of three (3) years of experience as a Registered Nurse in an acute care setting
Demonstrated knowledge of medical terminology and desktop software, including Microsoft Office
Preferred
Bachelors Degree in Nursing (BSN) (BSN may be the minimum requirement at some care sites)
At least three (3) years of experience in a case management role
Physical Requirements:
Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$41.22 - $63.59
