Intermountain Health Social Worker in American Fork, Utah

Job Description:

The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.

  • Shift:Shift days and times vary. Emergency Department is staffed 7 days per week. Shifts are 6- and 12-hour increments during the early morning, day, evening, and overnight. All team members have night, weekend, and holiday shift requirements. PRN staff are required to work a minimum of one shift per week with the opportunity to work additional shifts at their discretion.

The Emergency Department Social Work Care Manager is part of a team providing services, resources, and clinical intervention to a variety of patients and families. Our role in the emergency room is to respond and provide trauma intervention, grief and loss support, general social work and community resourcing, drug and alcohol assessment and placement, care management, utilization initiatives and readmission goals, and mental health/emotional support.

This PRN position supports our emergency departments at Utah Valley Hospital, American Fork Hospital, Riverton Hospital, and Alta View Hospital. Shift times would vary and are available 7 days a week in 6- and 12-hour increments during early morning, day, evening, and overnight hours. All team members have night, weekend, and holiday shift requirements. PRN staff are required to work a minimum of one shift per week.

Job Essentials:

  • The Emergency Department Social Work Care Manager works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs.

  • ED SWCM identifies patient populations using specific screening criteria including psychosocial screen, medical record review, or through referrals for service per department policy/guidelines. Obtains additional necessary information by interviewing the patient/family/significant other. Through clinical assessment, ED SWCM may diagnosis various mental, emotional, and behavioral disorders using DSM 5 guidelines. ED SWCM provides supportive, crisis, and other social work interventions as identified. ED SWCM may provide therapeutic modalities appropriate to the level of care and acts within the scope of the Mental Health Practice Act. ED SWCM coordinates and collaborates with external agencies including DCSF, APS, and law enforcement when abuse, neglect or exploitation is suspected or confirmed.

  • ED SWCM assesses patient per CMS Condition of Participation (COP) or National Clinical Quality Association (NCQA) standards and ensures a plan of care is developed and maintained for patients receiving care management services consistent with COP/NCQA and department standards. ED SWCM assesses patients for post-acute needs and referrals; actively intervenes and resolves/escalates where barriers to service exist across the care continuum; maintains a current knowledge of community resources; and promotes optimal allocation and coordination of these community resources while balancing clinical quality and cost management.ED SWCM provides education to patients and caregivers to help them manage their health care needs.

Minimum Qualifications

  • Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.

  • Current State Licensure as a Licensed Certified Social Worker (CSW) or Clinical Social Worker (LCSW).

  • Basic computer skills and knowledge of word processing and spreadsheet software.

  • Strong written and verbal communication skills.

  • Demonstrated ability to exercise critical thinking skills.

Preferred Qualifications

  • Case Management Certification.

  • Experience working in clinical care management, social work or with third party payers.

  • Knowledge of care management.

  • Knowledge of health insurance related products.

  • Working understanding of coding, episode of care and length of stay guidelines.

  • Written and verbal communication skills.

  • Ability to work independently, be self-motivated, have a positive attitude, and be flexible in a rapidly changing environment.

Physical Requirements:

  • Interact with others requiring the employee to communicate information.

  • Operate computers and other office equipment requiring the ability to move fingers and hands.

  • See and read computer monitors and documents.

  • Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
